True Orthodontic Emergencies: Seek Care Immediately
These situations require a same-day or next-day appointment with your orthodontist or, in rare cases, an emergency dental clinic:
- Trauma to the face or mouth that has displaced teeth or damaged an appliance during a fall, accident, or sports injury
- Severe, unmanageable pain that is not relieved by standard over-the-counter analgesia
- Swallowed or aspirated hardware – if you think you may have inhaled a bracket, wire, or elastic, go to a hospital emergency department immediately
- Infection or abscess – swelling, fever, and severe pain around a tooth suggest infection, which requires urgent dental assessment
- Lower front teeth biting into the upper palate (traumatic deep bite) causing tissue injury
Non-Emergency Orthodontic Problems: Manage at Home, Then Call Us
Most orthodontic “emergencies” are actually discomfort or inconvenience that can be temporarily managed at home until a regular appointment. Here is a guide to the most common situations:
Poking or Broken Wire
A wire that has slipped out of the last bracket or has broken can poke the cheek or gum. What to do:
- Use a blunt pencil eraser or a cotton bud to gently push the wire toward the back tooth, away from the soft tissue
- Cover the sharp end with orthodontic wax (available at any pharmacy)
- If the wire is long enough, you can carefully cut it with small nail scissors or fingernail clippers – sterilise first with alcohol
- Call Orthoworx for an appointment at your earliest convenience
Loose or Detached Bracket
Brackets can come loose if you eat hard or sticky foods, or if there is a knock to the mouth. What to do:
- If the bracket is still on the wire, leave it in place – cover it with wax if it is irritating
- If the bracket has come off the wire entirely, remove it safely and bring it to your appointment
- Call Orthoworx to schedule a repair appointment – a detached bracket delays your treatment if left too long
- Do not try to re-bond the bracket yourself
General Soreness After an Adjustment
Mild to moderate aching for two to five days after a brace adjustment or a new set of Invisalign aligners is completely normal. This is your teeth and periodontal ligament responding to movement. What to do:
- Take your usual over-the-counter pain relief (ibuprofen or paracetamol) as directed
- Stick to soft foods for the first few days after an adjustment
- Salt-water rinses soothe irritated gum tissue
- This is not an emergency – no appointment is needed unless pain is severe or persists beyond a week
Lost or Broken Retainer
- Call Orthoworx promptly to arrange a replacement retainer appointment
- If your previous retainer still fits but is cracked, wear it until your appointment if it is not sharp or uncomfortable
- Do not try to repair a retainer with household adhesive
Broken or Lost Orthodontic Plate
- Stop wearing it immediately if it is sharp or no longer fitting correctly
- Bring all the pieces to your appointment – the laboratory may be able to repair some types
